Each of our Assisted Living and Memory Care residents receives an individual care plan overseen by a registered nurse, which is updated regularly to meet their evolving needs and preferences. Independent Living residents have peace of mind knowing they can gain access to a care plan and services if needs evolve.
For those with higher acuity needs, The Preserve offers advanced nursing services, including medication management, specialized therapies, mobility assistance, and collaboration with healthcare professionals to address specific medical conditions. Residents can comfortably age in place at The Preserve knowing their needs will always be met no matter how they progress over time.
In addition to our outstanding, compassionate care, our Available services include:
- A thorough assessment by a registered nurse (RN) to create an individualized care plan for each Assisted Living and Memory Care resident. Our team at The Preserve of Roseville works closely with your physician, you, and your family to develop an individualized care plan to fit your needs best.
- Assistance with activities of daily living provided by trained staff—bathing, dressing, grooming, hygiene, and transfers
- A la carte nursing services—residents select only the services they want and need
- Monthly vitals to monitor health status and modify personal care plan as needed
- Open and regular communication with family members by licensed staff members
- Registered Nurse on-call during nighttime hours
- Secure memory care unit with high staff-to-resident ratio
